Who is eligible?

Women up to age 44 seeking to conceive their first or second child with their current partner (regardless of any children from previous relationships).

Prior approval from a Maccabi medical professional is necessary.

What does Maccabi provide?

IVF treatments for a first or second child in three phases:

Phase I - Consultation, tests, and hormonal treatment (ovarian stimulation).

Phase II - Egg retrieval, fertilization with sperm, and embryo transfer to the uterus, optionally involving micromanipulation. Unused embryos may be frozen for preservation.

Phase III - Thawing a frozen embryo from Phase II and transferring it to the uterus.

Maccabi members up to age 53 (until their 54th birthday) may choose for a frozen embryo from Phase B to be thawed and transferred to the uterus, provided that Phases A and B were completed by age 44 (before their 45th birthday).

Maccabi Sheli, Maccabi Zahav, and Maccabi Silver Shield members are eligible to select a private doctor at private hospitals affiliated with Maccabi.

Sperm retrieval - In certain cases, the second part of the fertilization process may necessitate sperm retrieval.

How much does it cost?

At private hospitals, with the option to select a doctor under agreement (or from the provided list below):

The authorization will be issued by Maccabi's national center for fertility treatment approval, which will directly cover the deductible at the hospital as outlined below:

Phase I - deductible of NIS 424.93

Phase I + II - deductible of NIS 902.30

Phase III - deductible of NIS 264.92

At public hospitals under agreement, without choosing a doctor:
Outpatient clinic visits- a quarterly deductible of NIS 38.

IVF treatments - Free of charge. The cost of fertility drugs is not covered.

Basic Plan

A financial authorization will be issued with a quarterly deductible of NIS 38.

Eligibility does not cover the expenses for purchasing fertility drugs.

Next steps

Request approval for fertility treatments

Prior approval from a Maccabi medical professional is necessary. The number of treatment cycles will also be determined based on the discretion of the professional.

For the first treatment - please visit a licensed fertility doctor. It is their responsibility to complete a designated form and send it directly to Maccabi's national center for fertility treatment approval.

To undergo blood tests for fertility treatment, a referral from a Maccabi-licensed fertility doctor, hospital, or private doctor is necessary.

Treatment in a public hospital requires an authorization for a doctor's exam.

For follow-up treatments - it is the doctor's responsibility to complete and submit a follow-up treatment form to Maccabi's fertility treatment approval center.

For treatments at public hospitals, a file must be opened at the hospital, and an authorization form for a doctor's exam must be provided before initiating the procedure to request approval for fertility treatments.

You will receive a response from the fertility doctor who submitted the request.

You can track the treatment status either via Maccabi Online, or by signing up for texts.

Purchasing fertility drugs

  • Eligibility does not cover the expenses for purchasing fertility drugs.
  • You can purchase fertility drugs through age 44 (up until your 45th birthday).
  • Before contacting the pharmacy to purchase medication, you must obtain financial authorization for treatment cycles A + B.
  • To use the medication approval at the pharmacy, you must print the approved authorization.
  • Medication approvals are valid for six months but validity shorten to 45 days after the first purchase.
  • Stage III (embryo transfer) - doesn’t involve medication. In exceptional cases requiring drug treatment, the fertility doctor must send a medical letter to the IVF fertility treatment approval center.
  • Quantitative control - Maccabi Pharm implements quantitative controls for fertility drug purchases.
  • If it’s necessary to dispense medication beyond the defined treatment amount, Maccabi Pharm will provide you with a designated form for your fertility doctor to complete, which you will then submit to the IVF fertility treatment approval center for confirmation.

Requesting an extension of medications for phases I + II

  • Within 7 days following the 45-day medication approval period, an original prescription signed by a fertility doctor is required. The prescription should specify that it is a continuation of treatment within the same cycle.
  • After 7 days following the medication approval period (45+7), it is the fertility doctor’s responsibility to provide the IVF fertility treatment approval center with a medical letter justifying the need to extend the prescription.

For women approaching 45

An authorization received from Maccabi at age 44 is valid only until the woman’s 45th birthday. After this date, purchasing the medications at a pharmacy and undergoing IVF treatment at a hospital will not be possible.

Additional information

    • Eligibility is also granted for a surrogacy procedure, with the condition that a medical approval signed by an authorized specialist be provided, verifying that the woman cannot conceive or pregnancy would endanger her health.
    • Regarding egg donation, eligibility for IVF phases is according to health services or supplementary health services regulations, as well as the woman's age.
    • If the patient's spouse is not a Maccabi member, their treatments will be financed by their own health insurance.
    • The authorization letter received from Maccabi at age 44 is valid only until the official date of the 45th birthday. After this date, purchasing the drugs at the pharmacy and undergoing IVF treatment at the hospital will not be possible.
